It is rare for any new applications, systems or software releases to be completely error-free. During the testing of any
new applications, the testing teams would identify defects which are passed on to the Delivery Team for further analysis
and resolution. However, it is quite possible, that a few defects are not rectified by the Delivery Team and are leaked
into production.
Whenever there is a release of a new application/system into production, the Problem Manager must connect with the Delivery
Team to obtain a list of defects that have leaked into production. If this proactive step is not taken and the user
experiences an incident or issues due to this problem in the production environment, the cost incurred to re-diagnose and
resolve this incident would be too high. Any defect that are released into the production environment must be logged and
tracked in a Production Defect Log.
